Hope for the Community (H4C)

We want everyone to have the tools to lead a hopeful and flourishing life
At Hope For The Community CIC social enterprise, we do this by providing affordable, co-created, evidence-based products and services to empower people to manage their health and wellbeing.
We work with people living with long-term conditions (cancer, autism, multiple sclerosis, depression) and their carers. We build their knowledge, skills and confidence to take an active role in their own care thus improving their quality of life and reducing the burden on health services.
Our flagship product, the Hope Programme ® is a six-week self-management intervention co-created with service users and healthcare professionals, based on over 20 years of research evidence from positive psychology, mindfulness and cognitive behavioural therapy.
The programme focusses on increasing hope and includes a range of evidence-based activities such as goal setting, gratitude diaries and activity scheduling. It is co-delivered by peer volunteers or healthcare professionals, digitally, virtually or in-person.
The Hope Programme is accredited by QIS2015 quality standard and licensed across the UK by NHS England, Macmillan Cancer Support and Carers Trust.
15,000 people with a range of mental and physical health conditions have benefited from our wellbeing products and services to date, feeling less anxious, depressed, stressed, more activated, grateful and hopeful.
As part of the mission, we wish to highlight the long-term advantages of self-management and are looking for partners to spread our programmes for the benefit of the ageing population in Singapore.
